AFYA GUEST TRAVEL HEALTH SURVEILLANCE FORM

By decision of the official authorities of Tanzania, all international passengers entering the country are required to fill out the Afya Guest Travel Health Surveillance Form before travel.

To ensure your travel goes smoothly, we kindly request you to consider the following points:

• Online Registration: Fill out the form at moh.go.tz before your travel.
• QR Code Presentation: Keep the QR code or reference number generated after form approval with you (digitally or printed) to present to Port Health officials upon arrival.
• Health Screening: Body temperature will be measured at entry gates, and passengers deemed at risk will be monitored via an automated messaging system for 21 days.

GENERAL NOTES AND WARNINGS FOR ALL OUR TOURS :
**Optional tours take place with a minimum participation of 20 people. In case of participation under 20 people, tour prices may change. If there are not enough participants, price differences that may occur will be informed by our guide before the tour after the numbers are obtained.
**Our guide may make changes to the days of the city tour and/or optional tours depending on the content of our tours, the number of participants, and the closing status of museums and historical sites. This is also valid in case of changes that may occur in flight times.
**Panoramic city tours do not include entrance to museums and historical sites and are intended to provide general information about the city.
**In triple rooms (3-person room: dbl bed + extra bed), the 3rd bed is an extra bed or sofa bed and is smaller than standard beds.
**Triple rooms (for 3 people) are provided subject to availability. If available, a 3-person room reservation will be made. Triple rooms are smaller due to the extra bed.
**Child discount is valid if staying in the same room with 2 adults.
**Check-in time at hotels is generally 14:00 (15:00 in some countries) and check-out time is 10:00. (09:00 in some countries)
**Group reservations are made at hotels for our programs. Content of group breakfasts may differ from individual breakfasts. Breakfasts to be served at hotels in visited countries are not Turkish breakfast. Group breakfasts are generally served as continental breakfast in a separate room. Continental breakfast basically consists of tea or coffee, marmalade, and croissant, and varies by country.
**Our guests participating in the tour without flights may not be able to use airport-hotel-airport transfers if they do not use the same flights as the group, and they may also not be able to attend the city tour. If they use a different flight, their transportation to the group's hotel is their own responsibility. This situation must be notified to Prontotour in advance.
**Baggage weight limit for most flights departing from Turkey is 20 kg. This weight may vary depending on the airline company and destination country. If there are domestic flights in the destination country, the baggage weight on these domestic flights may drop to 15 kg. Excess baggage weight/price rules are determined by the airlines and are not under Prontotour's responsibility.
**During registration, the name, date of birth, and passport number as stated in the passport must be entered into the system. Flight tickets are issued according to this information. Penalty costs of flight ticket cancellations or changes resulting from incorrect information will be reflected to the guests.
**Prontotour acts as an intermediary between our guests and the consulate and does not guarantee any visa issuance. The consulate's failure to grant a visa or rejection is not Prontotour's responsibility.
**Visa status in our programs is provided based on Republic of Turkey passports. Guests holding passports of other countries can check their visa status with the consulates of their respective countries.
**Passports must be valid for a minimum of 6 months based on the end of the travel date. Guests traveling with less than 6 months validity may face the situation of not being admitted into the country by country passport police. In such cases, responsibility belongs to the passport holder.
**In order to use your mobile phones abroad, before leaving Turkey, it is recommended to check with your GSM operator whether your phone is open to international use.
**Hotels far from the center may be used during special and busy periods such as fairs, congresses, and concerts.
**Some of our programs are offered based on hotels with several alternatives. Programs with multiple hotel options may share the same guide and transfer vehicle.
**In our tours labeled as Promotion, the location of our hotels and the content of the service provided may differ from our standard packages. Cancellations and changes cannot be made by the guest on our promotional tours. Please read the sales contract of the tour you purchased carefully.
**Guests who wish to upgrade flight tickets with their miles can perform upgrade procedures based on the availability of the airline (check the airline where your membership is located) after tickets are issued. Check whether the group tickets of the program are of upgradable class. Mileage is not guaranteed for every flight in our programs.
**Flight times specified in the programs are times reported by the airlines. On our tours organized with special flights, flight times may change up to 48 hours in advance. Prontotour acts as an intermediary between the airline and the guest. Prontotour cannot be held responsible in any way for delays, cancellations, airport and airline changes, overbooking, service disruptions, etc. originating from the airline.
**Road distances given in the programs are map-based. Travel times may be extended in situations such as traffic, weather conditions, geographical location of the visited country, road works and conditions.
**Food-beverage and online check-in services on some airlines may be subject to an extra fee.
